Providing consultation services through New Editions, Inc., to include quality assurance, technical assistance and oversight under the Money Follows the Person (MFP) demonstration for the Centers for Medicare and Medicaid.

Goal:
To facilitate state agencies to develop projects within their states utilizing the MFP project funds which will improve the quality of life of their states’ elder population with a focus on American Indian tribes within each state.

Working with our strategic partner, American Indian Health Management and Policy (AIHMP), complete a comprehensive operational assessment of the health center.

To improve the health center’s operations so as to provide improved health services for community members.

Fort Belknap Indian Community | Harlem MT

 
 

Project: 
Conduct three feasibility studies for a nursing home, assisted living and adult day health care. Provide the report for FBIC and for the project funder, the Economic Development Authority.

Goal:
To provide the FBIC with three usable feasibility studies which serves as a report for decision making purposes and as a resource for current and future projects.

Hopi Tribe - Hopi Assisted Living Facility Task Team | Arizona

 

Project:
Prepare and submit a comprehensive Sustainability Plan including licensure process for a new Assisted Living Facility.

Goal:
Provide a detailed plan that will facilitate the HALFTT board develop the necessary resources that will permit the financial operation of their new assisted living center. Assist in detailing the process and requirement for ‘licensing’ of the facility in Arizona. (It should be noted that Arizona respects tribal sovereignty and does not license facilities on Native Nation lands but does work out a process that permits oversight and third party reimbursement.)

Tohono O'odham Nursing Care Authority | Sells AZ

 

Project:
Multiyear agreement through 2016 to provide overall operational management services for the Archie Hendricks Sr Skilled Nursing Facility and Tohono O’odham Hospice. Planning and development of assisted living housing which is now in operation.

Goal:
Facility is fully staffed and occupied. Facility is well  received by tribal members. Successful Medicare  certification. Numerous awards including Highest Honors, Harvard Project on Native American Self Determination.

Indian Health Service | Washington DC

 

Project:
Working with our strategic partner, American Indian Health Management and Policy (AIHMP), we are engaged to analyze Indian Health Services practices and resources to reduce IHS hospital inpatient readmissions. Each of IHS 28 hospitals and respective tribal communities’ served will be evaluated on the basis of patient needs in order to identify additional health care services needed by IHS patients discharged from the hospital.   

Goal:
To reduce readmissions to Indian Health Service Hospitals and assist in developing recommendations for tribal based services so as to improve care transitions back to the community.
